Free NHS Right to Choose referral letter

Fill in a few details and we'll write a letter you can give to your GP asking for an NHS-funded ADHD or autism assessment with the provider you choose. It's free, with no sign-up. Not sure which provider? See your wait, explained or compare Right to Choose providers.

We don't store anything you enter. Your letter is made in your browser — the surgery search only looks up the public NHS GP list.

The picker opens around 2000 — set it to the patient's date of birth.

Don't know it? You can usually find your NHS number in the NHS App, on a letter from your GP or hospital, or in the online GP portal you use to contact your practice. You can leave it blank and add it by hand later.
